An aesthetic procedure that changed Alice’s life

UNDER YOUR SKIN - Grace Carole Beltran, MD - The Philippine Star

Alice is a 35-year-old lady who used to model for a living.  She got married when she was 23 to a man of Chinese descent.  She was very happy and contented with her married life for many years with her four children.  But after getting pregnant several times, she gained a lot of weight, developed bulges in all the unwanted places, and the worst thing was that her breast sagged tremendously.  Added to this was the belief that her husband was seeing another woman while she had to raise her four children completely on her own.  She receives an allowance from her husband, but that’s all.

Eight months before her 36th birthday, she decided it was the right time for her to improve herself and bring back the glory days.  On the day of her first consultation, she was quite anxious about showing her problems.  Finally, she mustered enough strength to tell all.

Of course, when they do aesthetic procedures, surgeons cannot promise to bring back philandering husbands.  What surgeons can only promise is an improvement that can make a lot of difference in a patient’s life.  In the case of Alice, improvement on her looks will improve her self-confidence and self-esteem, a transformation that can make a lot of difference.

Alice decided to undergo breast augmentation and some liposuction to remove fat rolls above and below her bra line and also to remove fats on her abdomen.  For breast augmentation, Alice was made to choose what shape she would prefer — does she want a round implant or a tear-shaped implant?  Also, does she prefer a smooth or textured implant? A smooth implant can move, with breast implant pocket giving a more natural movement, but it can have some palpable or visible rippling, too.  On the other hand, a textured implant makes the wound develop a scar that hugs the implant, making it less likely to move around, thus preventing repositioning.  Textured implants also diminish the risk of a contracture (tight scar around the implant) which could be painful.  She was also given a choice between a silicone and a saline breast implant.  Each type has the same basic implant anatomy — a rubbery silicone shell with filling. The difference is in the filling. Saline implants are filled with a saline (salt and water) solution, while silicone implants are filled with silicone gel.

Each type of implant has advantages and disadvantages. For example, silicone implants are considered more natural to the touch, but saline implants cost less.

Potential side effects of the two implant types also differ. For example, if the shell of a saline implant ruptures, the resulting effect is obvious because the saline drains from the implant (and is harmlessly absorbed by the body). However, if a silicone implant ruptures, the resulting effect may not be evident right away.  Also saline implants can produce funny swishing swashing sounds when running.  There are other high-end materials though these are quite expensive.

Alice underwent the necessary laboratory tests to make sure she did not have any medical problem that could compromise her safety for the procedure.  She was checked by a cardiologist for surgical clearance.

The magic day finally came.  She donned her surgical gown, after which her anesthesiologist asked her some questions as she proceeded to lie down on a T-shaped bed.  Her vital signs were taken and, finally, a mild anesthesia was administered.  In a few seconds, Alice was sound asleep, sedated but not unconscious.

Alice was awake right after putting the garments for the liposuction and bandages for her breasts.  She took a look at her breasts and was amazed at what she saw!  The third day was her first time to see her breasts without the bandage — they were awesome!  As for the liposuctioned areas, they were a bit inflamed yet so they still appeared big.  Weeks and months passed. Alice breasts began to soften and began to drop a little.  Her liposuction areas were not inflamed anymore so they appeared softer with the flabs gone.  She said her looks got better day by day.  She became more confident and more assured of a better life ahead of her.
